To make a claim, please click on the relevant button then download and save the fillable PDF form.
You will need Acrobat Reader to open and edit the form. If you don't have it, please click here to download for free.
For your convenience, downloaded claim forms are able to be edited, saved and submitted along with Full Clinical Notes and Invoices, using the Submit Claim button below. Or, if you prefer, you can email your completed claim form, full clinical notes and invoices to claims@petnsur.co.nz.
